The consultation inquiry form is hosted through Google Forms. Information submitted through the form may be processed and stored according to Google’s systems and privacy practices.

Data Retention

Inquiry information may be retained as long as reasonably necessary for communication, consultation preparation, administrative purposes, or legal compliance. You may request deletion of your information by contacting Pattern Shift.

Health and Crisis Information

Please do not use the inquiry form for emergency situations or acute mental health crises. Pattern Shift is not a crisis service, psychotherapy provider, or medical service.
